The Logan County Health Department is working with the Logan County Partnership to conduct a community health assessment.
The Logan County Partnership has been a cornerstone of community collaboration since the first meeting in 1989. Its mission is to improve the quality of life for Logan County citizens through sharing resources, networking, creating awareness, identifying gaps in services, and advocating. Logan County Health Department has been an active participant in this coalition since its inception.
The Logan County Partnership developed a five-year Community Health Improvement Plan in 2013. In preparation for developing a new plan, the coalition is conducting a new community health assessment. The data collected will guide the coalition in making informed public health decisions during its strategic planning process.
A “Community Themes and Strengths Assessment” is now available to the public. It is a survey that allows Logan County residents to provide input on a variety of community public health issues. We ask that anyone who lives and/or works in Logan County please consider completing this survey. It is completely confidential. Paper copies of the survey can be obtained from the Logan County Health Department. The survey can be taken electronically by going to the Logan County Health Department website and clicking on the link in the second item (underneath Vision, Mission and Values).
